UHN Connected Care Clinic (support for Healthcare Providers)
The UHN Connected Care Clinic launched at the start of the COVID-19 pandemic to provide timely access to care to help individuals get the care they need at home or in their community wherever possible. This new integrated model of care continues to grow its service offerings as information and treatment becomes available. The Clinic also provides critical clinical support and infrastructure surge capacity to support overall health system recovery.
COVID-19 Therapeutics
The University Health Network (UHN) & Women’s College Hospital (WCH) have partnered to support our community with in-person and virtual options to access COVID-19 treatments. This partnership supports those living in the City of Toronto – M postal codes.
1. I can do the clinical assessment for my patient myself
Determine if your patient is eligible for therapeutics see the Ontario eligibility criteria; for more information:

- For Paxlovid, see the Paxlovid: What Prescribers and Pharmacists Need to Know
- For Remdesivir see the Ontario Science Table Clinical Practice Guidelines
- Paxlovid (individual must receive within 5 days of symptom onset)
- Prescribe Paxlovid to be dispensed from one of the select community pharmacies. You can continue to use the UHN WCH Paxlovid Referral Form & Prescription as a prescription to community pharmacies, when necessary.
- Remdesivir (individual must receive within 7 days of symptom onset)
- If the patient is not eligible for Paxlovid, complete the Remdesivir Referral Form and email it to COVIDCare@uhn.ca.
2. My patient needs a VIRTUAL clinical assessment by another clinician

- Complete the General Referral Form and email it to COVIDCare@uhn.ca. Please provide as much information as possible
- The clinic is open Monday – Friday 9am – 5pm; Saturday 9am-12pm
3. My patient needs an IN PERSON clinical assessment by another clinician

- Make a referral to the WCH CCAC by calling (416) 804-4083 or after 4pm emailing CCACNavigator@wchospital.ca.
- Note: have the patient’s day of symptom onset, list of current medication(s), and their COVID PCR or RAT and eGFR results ready, if available.
- The WCH CCAC is open Monday – Sunday 8am – 4pm; 416-804-4083; After hours email CCACnavigator@wchospital.ca
- Note: If your patient doesn’t live in the City of Toronto (M postal code), please direct patients to a clinical assessment centre to receive a test, diagnosis, and disposition (including treatment with Paxlovid, if applicable).
- Make a referral to the WCH CCAC by calling (416) 804-4083 or after 4pm emailing CCACNavigator@wchospital.ca.
Summary of Resources
COVID-19 Care and Symptom Management (including Long COVID)
Most COVID-19 positive patients can be cared for at home. Our clinic provides virtual assessment and care, including access to pulse oximeters to help with remote monitoring. Individuals cared for will have a clinician follow-up and depending on needs may be followed over a period of time and offered additional supports as needed (e.g., social work, home nursing visit etc).
COVID-19 Testing, Vaccination and Health Screening
To ensure equitable access to testing, vaccination and health care and support services, community hubs are being established across the City of Toronto.
